Please Read - This form is to be used only when you are prepared to make an OFFICIAL OFFER to host one of the artist represented by The Harper Agency. Please do NOT submit an offer form before speaking with an agent regarding artist fee and availability. Once this form is received it is considered an official offer and will be submitted to artist management for approval. If accepted, a performance contract will be issued. The person submitting offer sheet MUST be at least 21 years old.
